Admission applications are available in downloadable PDF format below. In addition, some forms (as noted) may be submitted online. For additional information, please contact the Office of Admissions at 800-SEMINARY or 847-866-3945, or by email.
Please note that applications are due six weeks before semester of entry.
International students are normally only admitted in the fall semester. The deadline for international students seeking scholarships is March 1 of each year. The deadline for international students not seeking scholarships is May 1 of each year.

3. Personal Statement:
Your statement may be mailed to the office of admissions with your other material or you may send it as an This e-mail address is being protected from spambots. 4. Letters of recommendation coversheets:

6. Leadership Awards for Master's Degree Students - (optional)

Nomination deadline is January 15th of each year for students beginning their studies in the fall semester.
(Leadership awards are for full time students and are evaluated based on academic excellence, passion for ministry and leadership experience and promise. All students with an undergraduate GPA above 3.0 who are considering full-time studies are encouraged to apply. The Leadership Awards interviews take place in February and March of each year.)
